What is this medication?

INSPRA (Eplerenone)
Antihypertensive, Diuretic

INSPRA (Eplerenone) is indicated for treatment of hypertension, post myocardial infarction with reduced ejection fraction.

How does this medication work? 

INSPRA (Eplerenone) binds to the mineralocorticoid receptor and blocks the binding of aldosterone, a component of the renin-angiotensin-aldosterone-system (RAAS).  Aldosterone, a mineralocorticoid, increases blood pressure primarily by inducing sodium and water retention. Overexpression of aldosterone is thought to contribute to myocardial fibrosis (especially following myocardial infarction) and vascular fibrosis. Mineralocorticoid receptors are located in the kidney, heart, blood vessels, and brain. Eplerenone selectively blocks mineralocorticoid receptors reducing blood pressure in a dose-dependent manner and appears to prevent myocardial and vascular fibrosis.

How should I take this medication? 

Oral: Take 25mg to 50mg daily.

How to Take: Can be taken with or without food.

What should I watch for while using this medication? 

Before starting INSPRA (Eplerenone), make sure your physician is aware of any allergies or medications you currently take.  Eplerenone can cause hyperkalemia which is increased with renal impairment, proteinuria, diabetes, and patients taking concomitant angiotensin-converting enzyme (ACE) inhibitors, angiotensin-II receptor blockers (ARBs), nonsteroidal anti-inflammatory drugs (NSAIDs), and/or moderate CYP3A inhibitors. As a result, when taking Eplerenone  serum potassium levels should be closely monitored. Eplerenone should be used with caution in patients with pre-existing diabetes, heart failure and hepatic and/or renal impairment.

What if I miss a dose?

If you miss a dose of medication, try to take it as soon as possible. However, if it is almost time for your next dose, take only that scheduled dose. Do not take double or extra doses.

How should I store this medication?

Keep out of the reach of children at all times. Store at room temperature, 59 to 86° F (15 to 30° C). Protect from light. Keep the container tightly closed. Properly dispose of any unused medication after the expiration date.

What are the possible side effects of using this medication?

Hyperkalemia, hypertriglyceridemia, dizziness, fatigue, hyponatremia, gynecomastia, hypercholesterolemia, diarrhea, abdominal pain, cough.
