Antiretroviral, Protease Inhibitor (Anti-HIV)

KALETRA (Lopinavir/Ritonavir) is indicated for treatment of HIV-1 infection.

Note: Lopinavir/ritonavir is not recommended as a component of initial therapy for the treatment of HIV.

How does this medication work? 

Kaletra is an antiretroviral medication consisting of lopinavir and ritonavir.  Lopinavir and ritonavir are protease inhibitors that are used to treat infections with the human immunodeficiency virus (HIV).  Lopinavir binds to the site of HIV-1 protease activity and inhibits the cleavage of viral Gag-Pol polyprotein precursors into individual functional proteins required for infectious HIV. This results in the formation of immature, non-infectious viral particles. The ritonavir component inhibits the CYP3A metabolism of lopinavir, allowing increased plasma levels of lopinavir.

How should I take this medication? 

Oral: Lopinavir 400mg/ritonavir 100mg twice daily and/or lopinavir 800mg/ritonavir 200mg once daily.

Note: Once-daily dosing is not recommended in those receiving efavirenz, fosamprenavir, nevirapine, nelfinavir, carbamazepine, phenobarbital, or phenytoin.

How to Take:  May be taken with or without food. Swallow whole, do not break, crush, or chew.

What should I watch for while using this medication? 

Before starting KALETRA (Lopinavir/Ritonavir), make sure your physician is aware of any allergies or medications you currently take, and activities you perform. Lopinavir/Ritonavir has been associated with a number of adverse effects including altered cardiac conduction and prolongation of the QT and/or PR interval, accumulation of fat, development of immune reconstitution syndrome, and elevated cholesterol levels. Lopinavir/Ritonavir should be used with caution in patients that have an underlying structural heart disease, ischemic heart disease or cardiomyopathies, hepatic disease, pancreatitis, diabetes, and hemophilia A or B.

What if I miss a dose?

If you miss a dose of medication, try to take it as soon as possible. However, if it is almost time for your next dose, take only that scheduled dose. Do not take double or extra doses.

How should I store this medication?

Keep out of the reach of children at all times. Store at room temperature, 59 to 86° F (15 to 30° C). Protect from light. Keep the container tightly closed. Properly dispose of any unused medication after the expiration date.

What are the possible side effects of using this medication?
Increased gamma-glutamyl transferase, diarrhea, dysgeusia, vomiting, increased serum alanine aminotransferase, upper respiratory tract infection.
