What is this medication?

LORBRENA (Lorlatinib)
Antineoplastic Agent; Anaplastic Lymphoma Kinase Inhibitor; Tyrosine Kinase Inhibitor

LORBRENA (Lorlatinib) is indicated for the treatment of adult patients with anaplastic lymphoma kinase (ALK)-positive locally advanced or metastatic non-small cell lung cancer.

How does this medication work? 

Lorlatinib is a potent reversible third generation tyrosine kinase inhibitor.  It highly selectively targets ALK and ROS1, overcomes known ALK resistance mutations, and penetrates the blood brain barrier.  It is effective against multiple mutant forms of the ALK enzyme.  Lorlatinib also exhibits activity against TYK1, FER, FPS, TRKA, TRKB, TRKC, FAK, FAK2, and ACK.

How should I take this medication? 

Oral: Take 100 mg once daily and continue until disease progression or unacceptable toxicity.

How to Take: Follow instructions provided by physician/pharmacy label, take consistently at same times each day unless otherwise directed.  Administer with or without food at the same time each day. Swallow intact tablets whole. Do not ingest tablets that are broken, cracked or otherwise not intact.

What should I watch for while using this medication? 

Before starting LORBRENA (Lorlatinib), make sure your physician is aware of any allergies or medications you currently take, if you are pregnant, or breastfeeding. Lorlatinib can cause serious side effects including hepatic impairment and/or hepatotoxicity when used with other strong CYP3A inducers.  Lorlatinib can also cause CNS side effects leading to confusion, depression, speech impairment, hallucinations, and seizures.  Elevated cholesterol and triglyceride levels  have been associated with taking Lorlatinib. Lipid profile should be checked before treatment (baseline), 1 to 2 months after starting treatment, and during treatment.  Lorlatinib can cause bradycardia and arrhythmias.  The heart’s rhythm should be measured by an electrocardiogram before starting and during treatment with LorlatinibLorlatinib can cause severe and/or life-threatening peumonitis.

What if I miss a dose?

If a dose is missed, administer the missed dose unless it is due within 4 hours. Do not administer 2 doses to make up for a missed dose. If vomiting occurs, do not administer an additional dose, continue with the next scheduled dose.

How should I store this medication?

Keep out of the reach of children at all times. Store at room temperature, 59 to 86° F (15 to 30° C). Protect from light. Keep the container tightly closed. Properly dispose of any unused medication after the expiration date.

What are the possible side effects of using this medication?

Edema, peripheral neuropathy, cognitive dysfunction, fatigue, mood disorder, headache, dizziness, speech disturbance, sleep disorder, skin rash, hypercholesterolemia, hypertriglyceridemia, hyperglycemia, hypoalbuminemia, weight gain, increased amylase, hyperkalemia, hypomagnesemia, hypophosphatemia, increased serum lipase, diarrhea, nausea, constipation, vomiting, anemia, thrombocytopenia, lymphocytopenia, increased serum aspartate aminotransferase, increased serum alanine aminotransferase, increased serum alkaline phosphatase, arthralgia, myalgia, back pain, limb pain, visual disturbance, dyspnea, cough, upper respiratory tract infection, fever.
