What is this medication?

Propafenone, Rythmol
Antiarrhythmic Agent

Propafenone, Rythmol is indicated for treatment of paroxysmal atrial fibrillation/flutter (PAF) or paroxysmal supraventricular tachycardia (PSVT, atrial fibrillation

“Alert: US Warning
Avoid providing antiarrhythmic agents in patients with non-life-threatening ventricular arrhythmias, even if the patients are experiencing unpleasant, but not life-threatening, symptoms or signs.”

How does this medication work?

Propafenone/Rythmol is a class 1c anti-arrhythmic that is used to treat illnesses associated with atrial and ventricular arrhythmias. Propafenone, Rythmol blocks inward sodium current, and slows the rate of increase of the action potential. Prolongs conduction and refractoriness in all areas of the myocardium, with a slightly more pronounced effect on intraventricular conduction; it prolongs effective refractory period, reduces spontaneous automaticity and exhibits some beta-blockade activity.

How should I take this medication?

Usual (Oral):
Immediate Release: 150mg every 8 hours; increase dose every 3 to 4 days.  May increase to 300mg every 8 hours.
Extended Release: 225mg every 12 hours; increase dose every 5 days.  May increase to 425mg every 12 hours.

Note: Consider for use only in patients without structural or ischemic heart disease. Patients who exhibit significant widening of QRS complex or second- or third-degree AV block may need dose reduction.

How to Use:  Swallow capsule whole; do not crush or chew.  Can be taken without regard to food.

What should I watch for while using this medication?

Before starting Propafenone, Rythmol make sure your physician is aware of any allergies or medications you currently take, and activities you perform.  Propafenone, Rythmol has been reported to cause agranulocytosis, conduction disturbances (first degree AV block, PR interval prolongation, increase QRS duration), positive antinuclear antibody titers, hepatotoxicity, and drug-induced arrhythmias including ventricular fibrillation, tachycardia, asystole, and torsade de pointes.  Propafenone, Rythmol should be used with caution in patients with electrolyte abnormalities, heart failure, hepatic and renal impairment, myasthenia gravis, pulmonary disease.
